摘 要 目的: 探讨表皮生长因子受体(epidermal growth factor receptor, EGFR)基因和血管内皮生长因子(vascular endothelial growth factor, VEGF)基因mRNA在卵巢癌组织中的表达状况及临床意义。方法: 选取青岛大学医学院附属医院妇科手术治疗的58例患者新鲜组织标本(包括12例正常卵巢、11例卵巢良性肿瘤及35例卵巢上皮癌),用逆转录聚合酶链反应(RT-PCR)技术检测EGFR mRNA和VEGF mRNA的表达情况,分析它们与恶性肿瘤临床病理特征的关系及它们之间的相关性。结果: 卵巢上皮癌组织中EGFR、VEGF mRNA阳性表达率(71.4%、100%)均显著高于正常卵巢(16.7%、25.0%)及卵巢良性肿瘤组织(27.3%、36.4%)(P<0.05);EGFR mRNA表达与卵巢上皮癌手术病理分期有关,Ⅲ~Ⅳ期的阳性表达率及表达强度高于Ⅰ~Ⅱ期(P<0.05);VEGF mRNA表达与卵巢上皮癌手术病理分期及淋巴结转移有关,Ⅲ~Ⅳ期和有淋巴结转移组织的表达强度分别高于Ⅰ~Ⅱ期和无淋巴结转移组织(P<0.05);EGFR 和VEGF mRNA表达两者之间有显著相关性(r=0.438,P<0.05)。结论: EGFR和 VEGF基因高表达与卵巢癌的发生及转移有相关性,两者可能有协同作用。

Abstract Objective: To explore the expression of the epidermal growth factor receptor (EGFR) mRNA and vascular endothelial growth factor (VEGF) mRNA in ovarian carcinoma and its significance. Methods: Reverse transcription polymerase chain reaction (RTPCR) was employed to determine the expression of EGFR mRNA and VEGF mRNA in 58 specimens, including 12 normal, 11 benign and 35 malignant ovarian carcinomas, which were obtained during operation from patients treated in the Affiliated Hospital of Qingdao University Medical College. The relationship between EGFR and VEGF mRNA expression and the clinicopathological features was analyzed. Results: The positive rates of EGFR and VEGF mRNA were significantly higher in the malignant ovarian specimens than in the normal and benign ones (P<0.05). EGFR mRNA expression was correlated with the clinical stages of ovarian carcinomas, with the expression at stage ⅢⅣ significantly higher than that of stageⅠⅡ(P<0.05). The expression of VEGF mRNA was correlated with the clinical stages and lymph node metastasis of ovarian carcinomas. VEGF mRNA expression in carcinomas of stage ⅢⅣ or those with lymph node metastasis was significantly higher than that at stageⅠⅡ and those without lymph node metastasis(all P<005). Besides, the expression of EGFR mRNA was correlated with the expression of VEGF mRNA(r=0.438,P<0.05).Conclusion: The expression of EGFR and VEGF is positively correlated with the carcinogenesis and metastasis of ovarian carcinoma, and there might be a synergistic effect between the 2.
